Department Summary

HSC Health is the academic clinical practice for the Health Science Center. Focused on improving health outcomes to build a healthier community. HSC Health primary and multispecialty care clinics are located on the HSC campus in Fort Worth. Out care teams extend to local hospitals, surgery centers, and mobile and in-home care. HSC Health's interprofessional teams work collaboratively to achieve an optimized treatment plan tailored to the individual patient. Physicians, nurse practitioners, physician assistants, physical therapists, pharmacists, social workers, psychologists, specialized technicians, and nurses work together while intentionally integrating healthcare students, residents, and fellows to deliver care to our community.

Position Overview

The LVN position will support the Center for Older Adults (Geriatric Care). LVN will be responsible for telephone triage, providing guidance and support for medical assistants, medication management, inventory of clinical supplies, coordinating care across the practice, and other nursing tasks. This is an in person position only.

Minimum Qualifications

Graduation from an accredited school of nursing and two (2) years' of related experience. A valid Texas LVN license and a current CPR (BLS) certificate are required.

Knowledge, Skills and Abilities

* Knowledge of medications, their use, and means of administration
* Customer Service Oriented
* Practical working knowledge of nursing procedures, methods, and techniques
* Ability to perform nursing duties while standing or walking for long period of time
* Knowledge of safety and security precautions appropriate to work performed

Preferred Qualifications

The preferred candidate will possess the following additional qualifications:
* Bilingual (English/Spanish)
* Previous experience with older adults including telephone communication
* Ambulatory care experience
* Familiarity with adult vaccine requirements
* Patient rights
* Medicare and Managed Care experience
* NextGen EMR experience

Required License/Registration/Certifications

Graduation from and accredited school of nursing
2 years nursing experience
Active and unencumbered LVN license in the state of Texas
Current BLS (CPR for healthcare workers) certificate

Job Duties:

Measure and record patients vital signs, perform procedures and administer medications, injections and vaccines as ordered by clinician in the electronic health record. Assembles, maintains, and properly uses equipment in patient care delivery.
Return patient calls, and performs telephone triage following standardized protocols and clinicians guidance.
Gauges patients overall health status updating clinicians as indicated. Provides oversight and guidance to medical assistants in collaboration with clinic supervisor/manager.
Obtain insurance authorizations for referrals, procedures, and medications. Follows clinical protocols for medications refills. Completes patient forms as necessary.
Follows all departmental and organizational policies and procedures with regard to patient safety, quality management, infection control, and human resources. Ensure appropriate legal and regulatory standards
Coordination of care with internal and external partners.
Other duties as assigned.
