Summary
The Nursing Manager position oversees Healthcare for the Homeless – Houston’s permanent supportive housing program and is responsible for maintaining a patient focused practice environment to drive progress and achieve goals in the areas of patient experience, quality, access, employee engagement, care coordination, patient and employee safety, EHR regulatory compliance, e-Health and other initiatives. This position provides direct patient care for a small, assigned patient-panel of formerly homeless patients and supervises up to ten direct reports of Community Health Workers and RNs who work across five permanent supportive housing sites. This position reports to the Director of Clinical Operations and collaborates closely with the Chief Medical Officer and Director of Programs and Outreach. This position also serves as the main point of contact and liaison to coordinate services with community partners working jointly within the supportive housing sites. This position requires competency working in community-based health with patient who are vulnerable and have complex health and social needs, including high prevalence of chronic health conditions, mental health and behavioral health diagnoses and histories of chronic homelessness.
Job Duties
- Model the Mission, Vision, Values, Code of Ethics and Standards of Excellence of Baylor College of Medicine and Healthcare for the Homeless – Houston (HHH).
- Supervise a staff of Community Health Workers and RNs within supportive housing programs.
- Ensure that practice operations and team members are knowledgeable of and in compliance with applicable regulatory, licensing and accrediting agency standards
- Ensure care teams operate based on standard best practice with a patient centric workflow while maintaining high standards of care.
- Recognize and act on opportunities for growth and improvement.
- Translate goals into departmental specific objectives and priorities.
- Implement communication strategies and promote effective communication dialogue with providers and staff through regularly scheduled staff meetings and other means of communication.
- Participate in applicable meetings and oversee permanent supportive housing nursing care management-related issues, including assist in leading frequent permanent supportive housing leadership team meetings.
- Regularly meet with the permanent supportive housing nurses to review ongoing site concerns, growth, and areas of improvement.
- Monitor nurse performance metrics; research best practices to continue to improve the nurse-patient relationship.
- Manage clinical employees, to include defining work responsibilities and prioritization, setting performance development plans and goals, and conducting annual performance evaluations.
- Analyze data with director of clinical operations and physician co-leader, using data to work collaboratively with providers, other team members, patients and vendors to continually enhance performance and positively effecting clinical outcomes and patient satisfaction.
- Actively initiate and monitor plans for onboarding new staff joining designated site(s).
- Effectively implement and monitor utilization of EPIC for improved preventive medicine and disease management practices.
- Assume responsibility for the recruitment, onboarding, and orientation of qualified clinic managers and staff.
- Manage a practice by improving utilization of resources and maintain practice efficiency while seeking to minimize operational costs.
- Oversee and evaluate clinical equipment, supply, and pharmaceutical needs and request.
- Oversee the daily operations of the practice with the physician Chief Medical Officer to include: clinical work-up, treatment and flow of patients, capture of clinical information in EPIC, and proper coding and charging of services.
- Monitor key performance indicators for trends within the designated area(s) and implement prompt changes in process(s) when required.
- Work in collaboration with providers, and designated team members in practice setting and corporate support areas in preparation of internal and external reporting, budgeting, forecasting, capital budgeting, and financial modeling.
- Primarily responsible for delivering an outstanding patient experience for every patient every time. Ensure clinic maintains minimum overall Press Ganey mean score rank standard as set by Healthcare for the Homeless – Houston.
- Work with physician leadership and business development to evaluate and develop strategies for growth and outreach.
- Attend department and staff meetings.

Minimum Qualifications
- Bachelor's degree in Nursing.
- Four years of relevant experience as a licensed Registered Nurse (RN).
- Current license as a Registered Nurse (RN) by the State of Texas Board of Nursing.Basic Life Support (BLS) Certification through the American Heart Association.
Preferred Qualifications
- Two or more years of supervisory/management experience
- Ability to use Microsoft Office and Excel, spreadsheets, and computer data-base systems
- Familiarity with Electronic Health Records, EPIC preferred.
- Experience working within Federally Qualified Health Center(s) or other community-based health setting.
- Experience working with patients who have co-occurring physical and behavioral health conditions and/or homeless patient populations.
