Job Description

Summary

This is a part-time, year-long patient navigator position in which the patient navigator will be working closely with patients enrolled in a research project to educate them about improving health in pregnancy and after delivery and acting as a liaison between the patient and their case management team. The patient navigator should be enthusiastic about sharing their lived experience with patients who are passing through similar life journeys. the applicant should be able to speak, read, and write in both English and Spanish.

Job Duties

  • Identifies eligible patients.
  • Assists patients with gathering information for application.
  • Collects metrics and data on participating patients.
  • Accompanies and supports patients through the transition process.
  • Provide resources to appropriate support groups for patients and families.
  • Coordinates scheduling appointments, referrals, transportation, and other needed services.
  • Monitors and documents appropriate transfer of patient information.
  • Creates Standard Operating Procedures (SOPs) and recommends process improvements.
  • Attends and contributes to work related meetings.
  • Collaborates with patients, providers, and staff.
  • Performs other job duties as assigned.

Minimum Qualifications

  • High School diploma or GED.
  • Four years of relevant experience.

Preferred Qualifications

  • Bilingual in English/Spanish.
