Job Summary
The position will complete and oversee a variety of assignments at the discretion of the Chief of Social Work. This position will be responsible for collecting factual information from clients, families, and community agencies. This individual will coordinate with staff in the development, implementation and evaluation of individual treatment plan. This position will gather information in regards to the specific services available to each specific client and will communicate this to family and treatment team. This position will also be available to interview, assess, and represent clients in order to meet their specific needs. Screening, developing, monitoring, and coordinating placement plans will be a major responsibility of this position.

Responsibilities
Responsibilities include, but are not limited to:
Conducts psychosocial assessments to aid in diagnosis and treatment planning.
Completion of documentation, monitors, evaluates and records clients progress through individual sessions, interviews, and review of patient records.
Provides counseling/therapy as directed by treatment plan to include family sessions.
Discharge planning and case management services. Completes documentation for referrals to Community services.
Acts as Hospital Access Coordinator for Individuals with language needs and Sensory Impairment.
Assists in scheduling coverage.
Acts as individuals advocate to coordinate required services.
Court Testimony on civil and outpatients commitments.
Case Presentations/Case Formulations.
Development of Safety and Crisis plans.
Prepares reports for Hospital Leadership/Social Work Chief.
Participates in community collaboratives, weekly liaison calls.
Participates in Social Work Council at East Central Regional Hospital, assists in the development of policies and procedures.
Performs other related duties as assigned.

Required Qualifications
Educational Requirements
Master's Degree in Social Work or Counseling from an accredited graduate school accredited by the Council on Social Work Education (CSWE) w/licensure eligibility and two years of related work experience. Knowledge of diagnostic impressions or criteria and client advocacy systems. Ability to perform crisis intervention techniques and individual and group therapy technique. Skilled in a variety of therapeutic models and interventions as directly related to area of assignment. Ability to maintain confidentiality.

Proficient in Microsoft Office and other computer software/databases

Shift/Salary/Benefits
Shift: Days; Monday - Friday (Work outside of normal business hours may be required)

Salary to be commensurate with qualifications of selected candidate within the established range (generally minimum-midpoint) of the position
