Nurse Case Manager

 

This is a part-time position at approximately 20 hours/week.

.50 FTE

 

Skills/Duties:

Candidate must have excellent communication skills, organizational skills, problem solving skills, interpersonal skills and is accountable to resolve issues within the scope of the position.

Nurse Outreach & Facilitator:

Responsible for all aspects of coordination and facilitation of the pre-conception shared medical visits.  In coordination with the clinician, the nurse facilitator will help to recruit, retain and help participants meet their goals during the 6 weeks of group classes.  Evening hours may be required.  Must have experience with group facilitation, chronic disease management, and self-management support.

  • Engage with community organizations to recruit participants.
  • Facilitate preconception education classes.
  • Work with physician champion to augment curriculum.
  • Work with Tulsa Birth Equity Initiative to enhance curriculum and identify meaningful opportunities to collaborate. 
  • Work with community partners to successfully recruit class participants.
  • Work with individual patients to create self-management plans to help patients meet health goals.
  • Work in a culturally proficient manner that engages clients and work to meet their health goals.
  • Attend community events in order to build partnerships and coalitions within organizations working with targeted populations
  • Attend educational workshops / conferences that build skills in facilitation and ability to meet needs a clientele
  • Collect, organize, and report data on participants for research and quality improvement purposes
  • Help participants reach decisions and accomplish goals.
  • Work with groups to clarify meeting content, objectives and to establish ground rules that govern behavior.
  • Select appropriate methods and tools that will enable participants to achieve results. As problems or issues arise, facilitator will work with the group to resolve conflict and return to goals.
  • Create environments that allow civil dialogue and input from all participants and above all - remaining neutral throughout the process.
  • Requires excellent communication, listening and social skills.
  • Requires skills in group learning techniques, as well as skills in problem solving, conflict resolution, team building, planning, and organizing.

Qualifications:

 

Working Conditions:

  • Physical: Sitting for long periods of time. Speaking and listening.
  • Environmental: Office Environment.

 

Experience:

  • Required: 12 months of professional nursing experience.
  • Equivalent / Substitution: Will accept an Associate's degree in Nursing and 24 months of professional nursing experience in lieu of the Bachelor's Degree for a total of 36 months professional nursing experience.

 

Education:

  • Required: Bachelor's Degree in Nursing.

 

Certifications or Licenses:

  • Required: State of Oklahoma Registered Nurse Certification.

 

Knowledge, Skills & Abilities:

  • Knowledge of federal and state laws particularly patient confidentiality.
  • Knowledge of case management fundamentals and the technical/procedural aspects of case management.
  • Knowledge of medical terminology
  • Ability to resolve moderate to complex issues applying judgment, critical thinking and problem solving skills to determine a proper course of action.
  • Must have a high level of organizational skills in order to prioritize, organize, and complete projects or documents in a timely manner.
  • Ability to keep accurate, detailed records.
  • Ability to handle stressful situations and different situations and circumstances.
  • Ability to set priorities and manage obstacles while maintaining focus on goals.
  • Ability to build and maintain professional relationships with clients, colleagues, and other agencies while following ethical standards of practice.
  • Must be self-motivated to achieve results, be flexible when changes are necessary, and respond to feedback in a positive manner.
  • Ability to analyze information and evaluate results to determine the proper course of action while following established standards.
  • Ability to develop and maintain cooperative working relationships.
