Summary

The Sr. Coordinator, Care Navigation Services, serves as single-contact Care Navigator for high-touch Baylor VIP and alignment program patient populations, setting the tone for the optimal patient experience.  Coordinates registration, transfer of medical records and & specialty dept scheduling and testing. Must bridge communications efficiently between the client, physician, multi-department and clinical staff. 

 

The ideal candidate will have experience in resolving medical billing rejections and physician credentialing.  

Job Duties

  • Coordinates incoming premier appointment and testing requests for BCM, also BSLMC if needed.
  • Assists with sales and onboarding of patients within other alignment program service lines.
  • Triages patients for the appropriate specialty department and provider, coordinating services, dates and times. 
  • Completes EPIC registration and insurance verification. 
  • Acquires and uploads necessary medical records, images and insurance referrals.  
  • Orients patient to MyChart and telehealth visits, provides general wayfinding. 
  • Instructs patient on scheduling details and preparation for testing.
  • Assists with hospital transfers and direct admits. 
  • Proactively manages concerns and shares relevant information with care teams; rotates to front desk as needed.
  • Demonstrates the highest level of discretion and exemplary customer service.
  • Continuously develops innovative ways to optimize patient experience. 
  • Has excellent verbal and written communication skills and proven organizational abilities.
  • May include responding to after-hours calls 
  • High attention to detail and conscientious attitude to follow-up and complete enrollment details in two separate electronic platfoms - EPIC (clinical) and Salesforce (sales and membership). 

 

Minimum Qualifications

  • High School diploma or GED.
  • Four years of relevant experience.
